PBA provides "personalized" plastic identification cards (see sample below).

Many PPO's are now requiring names and coverage on ID cards. |

Some providers, hospitals and physicians have recently declined to accept ID cards that have no name or social security number. |

The "personalized" plastic ID cards provide the plan more security. The no-name generic cards can easily be used by anyone. |

PBA needs to be able to provide you with the type of ID card that the hospitals, physicians and PPO's are starting to require. |
